All students trying out for a school sport are required to complete and submit the following prior to the start of the school year:
A Sports physical ((Physical exams must be signed by a licensed physician, physician assistant (PA), advanced nurse practitioner or doctor before it is turned in to the coach or athletic trainer.)), a medical history form, and Athletic participation packet.
Student-athletes will not be allowed to participate in any athletic classes, practices or events until all necessary forms are completed and processed with the MISD Athletic Department.
